Complete Assessments During Home Healthcare Visits

As a clinician, you can gather patient responses for home healthcare assessments while performing home visits. Use the Field Service mobile app to complete assessments in both online and offline mode. You can also save assessments for later and resume them across devices. You can gather responses for multiple assessments at the same time with the help of Home Health's capability to autosave assessments. Save time, ensure uninterrupted care, and capture critical data even in low or no-connectivity environments.

          Required Editions

Available in: Enterprise and Unlimited Editions with Health Cloud, the Home Health Add-on license, and the Field Service mobile app
          User Permissions Needed
To complete assessments: Home Health Clinician

You must have the Field Service mobile app installed on your device.

          1. Open the Field Service mobile app.
          2. On the Schedule tab, tap a visit and review the visit details.
          3. To view the patient’s clinical details and to make the data available for offline mode, tap Actions and select Patient Details.
          4. Go to the Assessments tab and select the required assessment from the Library tab.
          5. Click Start and capture the patient's response for each question.
          6. To save your progress at any time, click Save for Later.
          7. To resume a draft, go to the Assessment tab and select the assessment from the In Progress tab.
            Note
Note Use the Assessment lightning component on desktop to resume drafts. Accessing a draft using a direct omniscript URL creates a new assessment instead of updating the existing draft.
          8. To get the latest data when resuming, select Refresh.
            Note
Note Images uploaded in the Field Service mobile app and saved for later remain accessible on your desktop in the Related tab of the assessment under Assessment Question Response (AQR). You can't edit or update these images after saving.
          9. After you complete the assessment, click Submit.
If you capture patient responses in offline mode, Home Health automatically syncs the data and creates the assessment related records when the mobile device becomes online.
