          • Request a Home Visit Reschedule
            As a patient, use Home Health’s patient portal to raise a request for rescheduling an upcoming home visit due to a change in your availability or preference. Request a visit reschedule at any time without a phone call or an in-person visit to the home health agency.
          • Request a Home Visit Cancellation
            As a patient, raise a request to cancel an upcoming home visit if you can’t make it to the visit on the scheduled date.
