patient portal. View your current, past, and upcoming visits at a glance, track visit duration,
and access care resource information.

To review visit details:
Home Health Patient permission set
Log in to the patient portal set up for your home health agency.
When the agency’s Salesforce admin creates your user account, you receive an email with
the login details and a link to reset the password. You can then use the new password to log in
to the site.
Review the visits for the current, past, and upcoming dates.
To search for a particular past or upcoming visit, click Filter,
provide the search criteria such as visit status, date, and work type, and then click
Apply.
Select a visit to view more information such as visit date, time, address, assigned care
resource, and their skills.
Click Call to call your home health agency or caregiver directly
from the site.
If your agency hasn’t enabled the Amazon Call Connect option in the site, you can call
them from your mobile device.
