As a patient, use Home Health’s patient portal to easily track and manage your home
visit requests. The portal provides a unified view that effectively categorizes your schedule,
reschedule, and cancel requests. You can also identify and review your submitted, approved, and
rejected requests at a glance. Get real-time status updates and automated notifications when the
schedulers at the home health agency approve or reject your requests.

To view home visit requests:

Log in to the patient portal set up for your home health agency.
When the agency’s Salesforce admin creates your user account, you receive an email with
the login details and a link to reset the password. You can then use the new password to log in
to the site.
Click My Requests.
Review your requests under the Schedule, Reschedule, and Cancel tabs.
You can see the request status, request date, visit date, and any additional comments. For
rejected requests, you can also see the scheduler’s reason for rejection.
