As a patient, take charge of your own healthcare by raising a request to schedule a new
home visit. Use Home Health’s patient portal on your personal device and book your visit as per
your availability and convenience.

User Permissions Needed
To request a new home visit:

Log in to the patient portal set up for your home health agency.
When the agency’s Salesforce admin creates your user account, you receive an email with
the login details and a link to reset the password. You can then use the new password to log in
to the site.
Click New Visit Request.
In the New Visit Request: Schedule window, provide your preferred visit date and time,
select the required service, and add any additional comments. If you’re requesting recurring
visits, make sure you add the recurring visit count and recurrence pattern as additional
comments.
You can only select the services that you’re eligible for. Reach out to the home health
agency if you don’t see the required services.
Submit your request.
Your scheduler at the home health agency reviews and approves your request, and then
schedules the visit. If your visit can’t be scheduled, your scheduler rejects your request and
provides a reason for the rejection. You receive an in-app notification indicating the approval
or rejection of your request.
