          Complete an Assessment to Use with Integrated Care Management

          Use Discovery Framework-based internal assessments configured in Salesforce, MCG assessments, and suggested assessments to gather health-related information from your patients or members. After you’ve collected that information, you can use it to create care plans.

          To complete an assessment:

          Read and Edit permission on Discovery Framework objects

          AND

          OmniStudio User permission set

          Only assessments with single-select or multi-select questions are compatible with Integrated Care Management. If you don’t have any assessments of this type, talk to your Salesforce admin.

          Assessments you complete from a Care Plan Activity page appear only on that page, not on the related Account or Contact page.

          1. Find assessments on a record page or an app home page.
            The account, care plan activity, case, and clinical service request pages show assessments related to a particular individual. The home pages of apps such as the Behavioral Health app, Integrated Care Management app, and Crisis Support Center Management app show assessments for all the accounts assigned to you.
            Note
            Note MCG assessments don’t appear on the care plan activity and home pages.
          2. To see all the available assessments, click the Assessment Library tab.
          3. Use the filter and sort buttons to find the assessment you need.
            If your admin has enabled MCG Care Guidelines, use these filters to find the assessment you’re looking for:

            Search Assessments In: Select Internal Assessments to search Discovery Framework assessments configured in Salesforce, MCG Assessments for external assessments that are based on MCG guidelines, or Suggested Assessments for secondary assessments suggested from MCG and internal assessments.

            Search Type: Specify the criteria for your search.

            Product: Select TC if the assessment is based on Transition of Care guidelines. Select CCG for Chronic Care Guidelines.

          4. Click Start for the assessment you want to complete. For LIDM type MCG assessments, click New Care Plan to launch the care plan creation workflow.
            To complete an assessment that’s in progress, from the assessment’s action menu, click Continue.
            If a newer edition of an MCG assessment is available at the time of resumption, you can choose to start the new edition or resume the older edition.
          5. Ask the questions on the assessment and record the answers. If you’re administering MCG assessments, don't skip the questions tagged with QM as these are important for quality metrics reporting.

            If some assessment questions are prefilled with data from previously submitted responses, verify the information.

            If the assessment questions have suggested responses, click the suggested response icon, select the suggestion you want to apply, and click Apply Suggestion. To dismiss the suggestion, click Cancel.

          6. Click Submit when finished.

            If you can’t complete the assessment in one session, click Save for later to save your progress.

            To save your progress and continue with the assessment without interruption, click Save Changes.

            If autosave is enabled, the assessment is automatically saved every few seconds to make sure that you don't lose your progress.

            After you submit an assessment, it appears on the Completed tab.

            Open a completed assessment in view mode by clicking the Actions button, then View.

          After submitting MCG and internal, Discovery Framework assessments, patients may be suggested optional, secondary assessments based on their responses to the primary assessment. Administer suggested assessments to patients to thoroughly examine any potential risks or comorbidities in a timely manner.
