          Create an Enhanced Care Plan from Assessments

          Create care plans from completed assessments using an OmniScript flow. When you use this work flow, Integrated Care Management gives you recommendations for what problems, goals, and interventions you should add in the care plan.

          Industries Assessment permission set license
          To create a care plan Create permission on Care Plan and Case
          Read and View All Records permissions on Care Plan Templates, Care Plan Template Problem, Care Plan Template Goal, Problem Definition, Goal Definition, Problem Goal Definition, Action Plan Template
          To add a condition Create permission on Health Condition
          Read and View All Records permissions on Care Plan Template Problem, Problem Definition, Goal Definition, and Problem Goal Definition
          To add a social determinant Create permission on Care Barrier
          Read and View All Records permissions on Care Plan Template Problem, Problem Definition, Goal Definition, Problem Goal Definition, and Care Barrier Type

          Before you start, complete at least one assessment with the patient or member.

          1. Open a patient account, case, or clinical service request record page and navigate to the care plan interface.
            If you can’t find the care plan interface, talk to your Salesforce admin for help.
          2. Click New Care Plan.
            Tip
            Tip If you’re working with MCG assessments, you can also launch the care plan creation workflow by clicking New Care Plan on the assessment completion screen.
          3. Select Assessments as the care plan creation method.
          4. Click Next.
          5. Select the assessments you want to use for recommendations and click Next.
            Note
            Note If you’re creating a care plan from MCG assessments, you can select only one assessment.
          6. Enter a name for the care plan.
          7. Select a start date and end date.
          8. Specify the case number and the clinical service request name that you want to map to the care plan.
          9. Select the appropriate conditions, social determinants, goals, and interventions from the recommendations.
            Note
            Note To download SEND interventions from MCG, click the Download button next to the intervention. When you download a SEND intervention on the problems, goals, and interventions selection screen while creating a care plan, you can find the file in the person account’s related list. When you download a SEND intervention from the care plan interface, you can find the file in the care plan’s related list and your browser downloads.

            Note You can’t edit the labels and descriptions of the problems, goals, and interventions recommended by MCG.
          10. Save the care plan.

          You now have a care plan with some problems, goals, and interventions. In case you want to add more components to the care plan that weren’t recommended, see the next few pages.
