Care coordinators remotely monitor health metrics by viewing care observation data and remote monitoring charts on the patient’s account page. Remote monitoring charts provide biometric data and health metrics such as heart rate, blood glucose levels, and weight, which help care coordinators track and analyze patient health. - Manually Add a Care Observation
