          Remote Monitoring allows you to create a reliable and personalized connection with patients to motivate them on their healthcare journey. Learn how to create care observations, personalize care metric targets, track and analyze patient health data, and register and ship remote monitoring devices.

          With remote monitoring, you can:

          • Build trust with patients through more personalized, informed, and evidence-based interactions.
          • Employ auto-created tasks, care gaps, or cases when patient biometric data is out of range.
          • Spend more time giving useful information to patients than collecting data to identify problems.
          • Improve operational efficiency with fewer care coordinator calls.
          • View Care Observations and Remote Monitoring Charts
            Care coordinators remotely monitor health metrics by viewing care observation data and remote monitoring charts on the patient’s account page. Remote monitoring charts provide biometric data and health metrics such as heart rate, blood glucose levels, and weight, which help care coordinators track and analyze patient health.
          • Manually Add a Care Observation
            Care observations typically come from external systems. As a care coordinator, you can also add care observations manually. You can then use the remote monitoring charts to analyze the data and health metrics to track patient health.
