View Care Observations and Remote Monitoring Charts
Care coordinators remotely monitor health metrics by viewing care observation data and
remote monitoring charts on the patient’s account page. Remote monitoring charts provide
biometric data and health metrics such as heart rate, blood glucose levels, and weight, which
help care coordinators track and analyze patient health.

With remote monitoring charts, care coordinators can quickly identify healthcare gaps
and correlate care metrics over time. You can view healthcare statistics such as the target
completion percentage, average for the selected period, and change in care observations from
the past. When a patient’s biometric data is out of track, care coordinators can proactively
engage with patients by using autocreated tasks, care gaps, or cases.
View Care Observation Data by Using Charts
On the patient’s account page, in the Chart Metrics list on the Remote Monitoring
Charts component, select the chart metrics that you want to view. You can view up to 5
charts at a time.
Select the period for which you want to view the care observations and care metrics
target data.
Note The remote monitoring chart shows data for an account only
if the related care observation's Effective Date and Time is within the past 90
days.
View Care Observation Data Without Charts
On the patient’s account page, click the Related tab.
Click Care Observations and view the data.
Note Creating list views for care observations isn’t supported.
