Comprehensive Medication Review
Comprehensive Medication Review is a yearly, all-inclusive review of an enrolled patient. The structured and extensive evaluation process thoroughly analyzes all of a patient's active medications and relevant medical conditions. Patients are assigned to-do lists with action items to help improve their health outcomes.
After enrolling a patient in a Medication Therapy Management program, set up a one-on-one review of the patient’s medication regimen. With Comprehensive Medication Review’s 360-degree approach, you can help minimize the risk of medication-related problems by:
- Reviewing prescription and over-the-counter medications
- Detecting clinical issues
- Assigning to-do lists to the patient
- Tracking validations to ensure no medication is overlooked
- Sharing the Comprehensive Medication Review summary document with the patient after the review
- Periodically reassessing the patient’s health and addressing new issues
