Medication Therapy Management
Medication Therapy Management offers a broad range of healthcare services to help patients optimize their medication regimens for the best possible therapeutic outcomes and reduced healthcare costs.
Medication Therapy Management provides these benefits:
-
Personalized Therapy and Care
Optimize patient care by addressing each patient’s unique needs, based on their health concerns and list of medications.
-
Improved Outcomes
Greater medication adherence and improved control of chronic diseases reduces hospital admissions, and enhances patients’ quality of life.
-
Reduced Costs
Patients only pay for medications that are necessary for their health. Improved medication adherence and decreased adverse events reduce hospital admissions and overall healthcare expenses.
-
High Patient Engagement
Involving patients in discussions and decisions regarding their medical care allows them to take ownership of their health, promoting improved satisfaction and outcomes.
Patients are enrolled in a Medication Therapy Management program based on eligibility criteria such as count and cost of their medications, chronic diseases, and more. After they’re enrolled, patients can opt out of the program at any time.
The Medication Therapy Management program includes two review types:
- Comprehensive Medication Review
Comprehensive Medication Review is a yearly, all-inclusive review of an enrolled patient. The structured and extensive evaluation process thoroughly analyzes all of a patient's active medications and relevant medical conditions. Patients are assigned to-do lists with action items to help improve their health outcomes. - Targeted Medication Review
Targeted Medication Review is a focused review that identifies actual or potential problems with specific medications in the patient’s medication regimen. The review’s ongoing monitoring approach is a cost-effective option that helps to identify and proactively prevent or resolve medication issues.
