Partnering with patients and helping them play an active role in their healthcare is
essential in comprehensive medication reviews. By identifying a patient’s issues and assigning
patient-specific action items, you can help to mitigate the issue, enhance patient self-care, and
improve patient satisfaction.
Required Editions
But, we know that it’s tedious to manually create an action plan during a review. That’s why
we’ve integrated Medication Therapy Management with Care Plans to give you predefined tasks for
every issue. You can also add tasks to provide customized care and meet the individual needs of
your patients.
During a medication therapy review, use a to-do list to record your conversation with the
patient, highlighting the issues that you discussed and action items that you recommend. These
details are shared with the patient as part of the comprehensive medication review summary
document.
