          Medication Reconciliation

          With more than 40 percent of medication errors believed to result from inadequate medication reconciliation, it’s critical that you perform a thorough medication reconciliation during transitions of patient care. Medication reconciliation is a comprehensive evaluation of a patient’s medication regimen achieved by comparing the patient’s current and previous medication lists. The process helps identify if the patient is taking any new medication, using non-prescription drugs, or has changed previous medications.

          Some examples of care transition points are:

          • Within a hospital (inpatient)
          • Discharge from hospital to home
          • From the hospital to a skilled nursing facility

          Medication Reconciliation is an add-on product feature that helps you manage the complete medication regimen of your patient. The Patient Medication Manager lightning component in Health Cloud provides an integrated medication reconciliation experience to help improve patient safety and reduce healthcare costs due to medication-related errors. By conducting a reconciliation using the Patient Medication Manager component, you can easily identify an accurate list of the patient's medications, including drug name, dosage, frequency, route, and more. With these details in hand, you can quickly spot and avoid medication errors such as omissions, duplications, and incorrect dosages.

          Based on your review of the medication list, you can add medication recommendations and detailed notes to guide patients. You can also add recommendations when there’s any discrepancy that could lead to drug adverse interactions, a need to change dosage and instruction of medication, and more. And you have the flexibility to add recommendations at three levels:

          • Specific recommendation for a particular medication
          • Common recommendation for multiple medications
          • Generic recommendation at the reconciliation level

          You can also review a list of a medication's recommendations from the previous medication reconciliation so that you can decide if any previous recommendations should continue.
