Targeted Medication Review is a focused review that identifies actual or potential problems with specific medications in the patient’s medication regimen. The review’s ongoing monitoring approach is a cost-effective option that helps to identify and proactively prevent or resolve medication issues.
Conduct Targeted Medication Reviews quarterly or whenever required, such as when a patient is discharged from a hospital or a new health issue is identified. You can conduct multiple targeted medication reviews for a patient simultaneously. On reviewing a medication or condition, record clinical issues to address your patient’s medication problems. As part of this continuous evaluation and review, you can periodically reassess your patients for changes in medication regimen, control of symptoms, and progress toward goals.
