Create Dynamic Assessment Questions
Create categories for logical groups of questions so that the questions are easy to find and use. Then create the actual questions for the assessment. If you’re setting up Dynamic Assessments for inspections, grantmaking, or investigative case management, relate the questions to regulatory codes and violation types.

Create Assessment Question Categories
Group assessment questions for different scenarios by using the Category picklist field. You use categories to search for questions when you add them to an Omniscript assessment form.
By default, we include two assessment question categories: Demographic and Financial. Create additional categories for inspections, complaints, and other high-level types of assessments. Or, create categories for specific types of assessments, such as Roof Inspection, Fire Safety Inspection, Electrical Inspection, Solar Permit, and Public Safety Complaint.
- In the object management settings for Assessment Question, click Fields & Relationships, and then click Category.
- In the Category Picklist Values section, click New.
- On separate lines, enter the Category values.
- Save your changes.
You can also use question sets to group questions by context. See Assign Questions to Question Sets.
Create Assessment Questions
Create questions to capture information about the site, conditions, circumstances, applicant, or situation you’re assessing.
- From the App Launcher, find and select Assessment Questions.
- Click New.

Specify these details.
- Enter a name and then press Tab to populate the developer name.
- Select one of these supported data types: Checkbox, Date, DateTime, Decimal, Integer, Multi-select, Radio, Select, Text, Text Area, or Time.
- Select a category.
- Enter the question text.
- For Radio, Multi-select, and Select fields, enter response values on separate lines.

Select Active.
Active questions can be used in Omniscripts.
- Save your work, or save it and create another question.
For a Dynamic Assessment for a salon fire safety inspection, create these two questions.
Question 1:
- Name: Area of Establishment
- Data Type: Decimal
- Category: Fire Safety Inspection
- Question Text: What is the square footage of the establishment?
- Active: Yes
Question 2:
- Name: Fire Extinguishers Present
- Data Type: Radio
- Category: Fire Safety Inspection
- Question Text: Are fire extinguishers present?
- Response values: Yes and No
- Active: Yes
Relate Assessment Questions to Regulatory Codes
If you’re using Dynamic Assessments for inspections, grantmaking, or investigative case management, relate your assessment questions to regulatory codes by creating regulatory code use records. Regulatory code uses are a junction between a set of assessment questions and the applicable regulatory code. A junction object creates a many-to-many relationship between objects. When you relate assessment questions to regulatory codes, the codes show with the questions in the omni assessment task, so the person who’s conducting the assessment can see the related code while they perform the assessment task.
To relate an assessment question to a regulatory code:
- From the App Launcher, find and select Assessment Questions.
- Select the question that you want to relate to a code.
- On the Assessment Question Versions related list, select the active question version.
- On the Regulatory Code Uses related list, click New.
- For Regulatory Code, search for and select the appropriate code. To create a regulatory code, click New Regulatory Code.
- Save your changes.
For a Dynamic Assessment for a salon fire safety inspection, create a regulatory code use record that relates the Fire Extinguishers Present question to regulatory code 19.05 Fire Code. If necessary, create these records.

- Name: 19.05 Fire Code
- Subject: Fire Prevention
- Description: A minimum of one portable fire extinguisher complying with Section 906 with a minimum 4-A rating or other approved onsite fire–extinguishing equipment.

Relate Assessment Questions to Violation Types
If you’re using Dynamic Assessments for inspections, grantmaking, or investigative case management, relate assessment questions to violation types by creating violation type use records. Violation type uses are a junction between a set of assessment questions and the applicable violation type. When you relate assessment questions to violation types, the violation types show with the questions in the omni assessment task, so the person who’s conducting the assessment can record violations that they observe while they perform the assessment task.
To relate an assessment question to a violation type:
- From the App Launcher, find and select Assessment Questions.
- Select the assessment question that you want to relate to a violation type.
- On the Assessment Question Versions related list, select the active question version.
- On the Violation Type Uses related list, click New.
- For Violation Type, search for and select the appropriate code. To create a violation type, click New Violation Type.
- Save your changes.
For a Dynamic Assessment for a salon fire safety inspection, create a violation type use record that relates the Fire Extinguishers Present question to violation type Fire extinguishers not present. If necessary, create a violation type record:
- Name: Fire extinguishers not present
- Type: Health and Safety
- Severity: Major Violation
