When you receive new information about a challenge that a patient or member faces, you
capture the information in a barrier card. Within the barrier record, you can select the type of
barrier you’re entering. Then, add important information such as the person’s name, the status and
priority of the barrier, the related case, and the effective dates. Barriers appear in the
component in the order you create them. So the newest barrier appears first.
Required Editions
Add the Care Determinants Component on the account’s record page using Lightning App
Builder.

From the account’s record page, scroll down to Barriers. In the search box, enter the name
of the barrier you want to find. If you can’t find it, you can add a barrier by clicking
New Barrier.
In Care Barrier Type, select an existing care barrier type. To create a care barrier type,
click New Care Barrier Type.
Enter a name for the barrier. This text appears as the heading of the barrier card or the
title of the barrier if your organization shows in a list.
Make sure the patient or member associated with the barrier is selected.
Select a status for the barrier. Your Salesforce admin can configure the app so that only
open barriers appear in the record.
Select a priority for the barrier.
To make the barrier type available to use, select Active.
Search for the case or the account that this barrier is associated with.
Optionally, select the date from when the barrier is no longer in effect.
Optionally, select the date from when the barrier goes into effect.
In Related Social Determinants, select at least one social determinant that is related to
the barrier.
To save the barrier and create related intervention tasks, click Save Barrier
and Add Interventions. To save only the barrier, click Save
Barrier.
