Provide Surveys for Gathering Feedback

Health Cloud Surveys help you create easy-to-use forms, such as pre-surgery surveys or member satisfaction surveys, for collecting feedback from your patients and members.

Customize questions and answers for your surveys in a simple editor. You can add various types of questions to gather the exact data you need.

For each patient or member, the Available tab lists the surveys you can send to that person. With a single click, the email invitation is on its way. When the survey participant clicks the survey link within the email, they can log into the site and complete the questions.

See all surveys you have sent to a person, along with survey status, date sent, responses, and other details on the Patient Surveys tab.

Note With Health Cloud, you have unlimited survey responses.
