            This template creates a service request record that captures essential user details for accurate and auditable fulfillment. Review what’s included with the template.

            Intake Attributes

            The intake form for this template captures these details from the employee:

            • Employee ID: The employee identification number.
            • Employee Name: The employee’s full name.
            • Preferred New Email Address: The employee’s preferred new email address.
            • Secondary Email: The secondary email address.

            Automated Fulfillment

            This service process includes a fulfillment flow that automatically processes the service request. You can extend this flow in Flow Builder to include custom logic, such as automated manager approvals or inventory checks.

            Integration
