Medication Management

Medication Management helps providers and care coordinators manage the medication regimen of their patients, helping to improve patient safety and reduce healthcare costs.

Note You must set up and configure Medication Review before working with Medication Management.

With Medication Management, your users can perform medication reconciliation and medication therapy review to improve care quality, safety, and patient experience.

• Medication Reconciliation helps providers and care coordinators reconcile their patients’ medications and add relevant recommendations during transitions of care.
• Medication Therapy Management helps healthcare providers and pharmacists to optimize a patient’s medication regimen for the best possible therapeutic outcomes and to prevent expensive drug-related problems. Medication therapy reviews are either comprehensive or targeted.

• Medication Reconciliation
  Medication reconciliation is a comprehensive evaluation of a patient’s medication regimen achieved by comparing the patient’s current and previous medication lists. The process helps to ensure health benefits and safety by identifying whether the patient is taking new medications, using non-prescription drugs, or has changed previous medications.
• Medication Therapy Management
  Medication Therapy Management offers a range of services to help patients optimize their medication regimen for the best possible therapeutic outcomes, reduced medication-related adverse events, and minimized healthcare costs.
• Medication Management Data Model
  Before you can use the Medication Management data model in your org, you must enable the FHIR R4-aligned Clinical data model and configure the Medication Review feature.
• Enable Medication Management and Assign Permissions to Users
  Enable Medication Management and assign the required permission sets so that providers and care coordinators can perform reconciliations of their patients’ medication regimen and medication therapy reviews.
• Configure Patient Medication Manager for Medication Management
  Add the Patient Medication Manager Lightning component to person account record pages so that your clinicians can start using Medication Management.
• Configure the Notes Tab in the Patient Medication Manager
  Enable care coordinators and providers to capture notes for each medication in the Patient Medication Manager.
• Create Care Programs for Medication Therapy Review
  Use Health Cloud’s Care Program feature to create a care program for medication therapy reviews for the corresponding year. Pharmacists can conduct comprehensive and targeted medication reviews for patients who meet eligibility criteria and are enrolled in the care program.
• Configure Reasons for Care Program Opt-Out
  Help your pharmacists record the reasons why their patients opted out of a medication therapy management care program. Patients can temporarily or permanently opt-out at any time, so it’s important to understand and track the reason why.
• Understand and Customize the Medication Therapy Review Flow
  The built-in Start Medication Therapy Review flow helps streamline the initiation process of comprehensive and targeted medication reviews by determining the patient’s cognitive status based on their health conditions.
• Set Up a Custom Flow for Medication Therapy Review
  Provide additional flexibility for your clinicians to start medication therapy review by using a custom flow instead of the built-in Start Medication Therapy Review flow. Depending on your business needs, you can set up a custom flow and associate it with the Patient Medication Manager lightning component.
• Create Predefined Tasks for Medication Therapy Management
  Help your pharmacists recommend to-do list items to their patients during medication therapy reviews. You can set up predefined, patient-specific tasks for addressing healthcare issues that pharmacists can assign to patients.
• Use Medication Management
  Health Cloud helps you reduce medication-related errors by providing you with the tools you need to develop a curated list of current medications; review medications with the patient; make clinical decisions and recommendations based on the review; and communicate the recommendations to the patient and the patient’s prescriber.
