          Unified Health Scoring

          Unified Health Scoring gives your users a holistic view of their patients’ health profiles. Using this feature, you can define a unified health profile for your organization that shows your users everything concerning their patients’ health. You can also configure contextual actions that prompt your users to intervene based on a patient’s score. This multidimensional view together with contextual prompts help your users plan their interventions better for improved health outcomes, greater patient engagement, and reduced healthcare costs.

          What You Get

          Here’s what you get with Unified Health Scoring:

          • Three metadata types
            • Score Category: Defines the different categories and subcategories of a unified health profile.
            • Score Range Classification: Defines and classifies the score ranges of a category or subcategory.
            • Score Category Calculated Insight: Maps a score category or subcategory to it’s calculated insight in Data 360.
          • Three standard objects
            • Health Score: Stores the score information for patients.
            • Health Score Action Log: Records the log of actions triggered from the Dynamic Actions for Unified Health Scoring component.
            • Action Visibility: Assists in defining contextual visibility rules for actions in the Dynamic Actions for Unified Health Scoring component.
          • Three lightning components
            • Score Details for Unified Health Scoring: Displays the historical score trends for a patient’s unified health profile and its subcategories. This information is also displayed as graphs.
            • Score List for Unified Health Scoring: Displays a list of patients with score changes in the last fifteen or thirty days and the degree of their score change.
            • Dynamic Actions for Unified Health Scoring: Displays a set of contextual actions your users can take based on the visibility rules you define for them.
          • Two connect APIs
            • Score Synchronization API: Updates score information in Health Score with the latest information processed by calculated insights in Data 360.
            • Synchronization Status API: Displays the status of a synchronization request.

          Integration with Data 360

          While Unified Health Scoring can render score information in an easy-to-interpret user interface, it can’t calculate that score information for you. That’s where Data 360 comes in. You can export a patient’s health records to Data 360 and use the Calculated Insights feature in Data 360 to process those records and derive scores. You can then import those scores back into Health Score object in Health Cloud. This integration allows you to define custom logic to derive scores using the data crunching capabilities of CDP.

          If you prefer not to use Data 360, you can always use custom solutions to derive health scores. But be sure to import those scores into the Health Score object.

          Important
          Important The use of certain sensitive data such as age, gender identity, or race, as well as others in the calculation of a Unified Health Score may introduce bias into your scores and predictions. We suggest you select these attributes thoughtfully, consistent with your organization's best practices for bias mitigation.
          • Unified Health Scoring Data Model and Permissions
            The Unified Health Scoring license gives you access to a set of objects, metadata types, and permissions that the feature requires.
          • Integrate Data 360 with Unified Health Scoring
            While Unified Health Scoring can display a patient’s score information and historical score trends in a user-friendly interface, it doesn’t calculate those scores for you. Use Data 360 to aggregate a patient’s related records and derive the required scores.
          • Unified Health Profile
            A unified health profile is the holistic view of a patient that covers the different aspects of their health and healthcare. This multidimensional view is achieved by defining a parent category for your profile and then defining all the subcategories that contribute to its performance.
          • Define a Unified Health Profile
            To define a unified health profile, create its score category, classify score ranges, and define subcategories. If you use Data 360 to calculate scores, also map its category and subcategories to their calculated insights in Data 360.
          • Configure the Unified Health Scoring Components
            Your users interact with Unified Health Scoring using its three lightning components. The Score Detail for Unified Health Scoring component shows users the detailed score breakdown of a patient’s unified health profile. The Score List for Unified Health Scoring component shows your users a list of their patients with their current and previous scores and the degree of score change. The Dynamic Actions for Unified Health Scoring component prompts your users to take action when certain conditions are met.
          • Customize Action Logs
            Actions triggered from the Dynamic Actions for Unified Health Scoring component are logged in the Health Score Action Log object. You can customize the information logged in this object using the IUnifiedHealthScore Apex interface in a custom Apex class.
          • Configure an Actions Timeline
            Use the Enhanced Timeline component to visually represent your users’ healthcare interventions with their patients or members chronologically.
          • Use Unified Health Scoring
            Unified Health Scoring gives you a holistic view of your patients’ health profiles. Let’s call this view the unified health profile. Your admin defines a unified health profile for your organization that shows you everything concerning your patients’ health. Based on how your admin implements Unified Health Scoring, the feature prompts you to intervene in your patients’ healthcare when certain conditions are met. This multidimensional view together with contextual prompts helps you plan your interventions better for improved health outcomes, greater patient engagement, and reduced healthcare costs.
