          Integrate Data 360 with Unified Health Scoring

          While Unified Health Scoring can display a patient’s score information and historical score trends in a user-friendly interface, it doesn’t calculate those scores for you. Use Data 360 to aggregate a patient’s related records and derive the required scores.

          Required Editions

          Available in: Enterprise and Unlimited Editions with Health Cloud
          Note
          Note You can use Unified Health Score without using Data 360, but you have to provide a custom solution that derives scores and imports those scores into the Health Score object.

          Data 360 uses data streams to pull information from Health Cloud and store that information in custom data model objects. You can then use Calculated Insights in Data 360 to aggregate that information and derive scores. And finally, use the Score Synchronization API to retrieve those scores from Data 360 and insert them into Health Cloud. But we’ll talk about that last part later.

          1. Connect Your Health Cloud and Data 360 Orgs
            To get started with your Data 360 integration, set up a Data 360 instance. Then, use the Salesforce CRM connector to connect Data 360 to Health Cloud.
          2. Configure Permissions for Data 360
            Configure the Data Cloud Salesforce Connector Integration permission set so that Data 360 can access your Health Cloud data.
          3. Set Up Data Streams and Custom Data Model Objects Manually
            If the Unified Health Scoring data kit doesn’t have a data stream you need, or if you’d prefer not to use the data kit, you can configure your own data streams and custom data model objects manually.
          4. Set Up Calculated Insights
            Calculated insights use the data stored in data model objects to run aggregates and derive scores. You can use either SQL queries or the Calculated Insights Builder to configure the score derivation logic.
