The Patient Segmentation dashboard lets you segment your patient population by risk, age group, and other factors. The Social Determinants dashboard metrics help you understand how social factors affect your patients.
Patient Segmentation
View the segmentation of your patient population according to risk, age group, number of conditions, and type of enrollment. See the column on right for top-level segmentation analysis, including a summary of risk score. It also shows average number of conditions per patient, social determinants impacting patient health, and age of patients least likely to adhere to their health plans. Click See Details in each chart for additional information about each demographic category. Scroll down to see a breakdown of patients by top 10 conditions and geography. And learn about each patient’s risk, conditions, care plan adherence, and social determinants in the Details table at the bottom.
Social Determinants
Understand how social factors and barriers standing between patients and health care impact your patient population. Filter the charts by time period, social determinants, barriers, and other factors to narrow the view of your population. Or, click a bar in the Top Social Determinants table in the upper left to view only patients affected by that determinant. The Population Breakdown chart in the upper right defaults to By Age. Click the tabs along the top to view the population breakdown by gender, location, or insurance type.
Charts further down show open barriers by priority and age and tasks by priority. Scroll down to the Details Table to see social determinants and barriers for each of your patients.
Visit Insights

Filter visits by time period, sales representatives, accounts, locations, and products for a targeted set of upcoming visits. Scroll down for a deep dive into each visit using the Visit Details table.
The Visit Accounts page contains account insights for upcoming surgical visits, top-selling products, top-performing sales representatives, and orders by status. Visualize account product whitespace analysis. Scroll down for a deep dive into each visit using the Visit Details table.

Get quick insight into revenue trends of your top contacts. Use the Revenue Overtime chart to learn about the impact of visits on revenue. See your top 10 accounts and top 10 products. The Team Performance page gives you key insights into the correlation between visits and the impact on overall revenue. Use time period-based filter selections on key revenue and visit metrics across your sales team.
