Care Gap Data Graph that joins data from the Unified Individual HECG DMO and other related
DMOs to get details to include in the outreach emails for care gap closure.
Unify Health Risk Details batch data transform that joins Health Risk Evaluation, Health
Risk Evaluation Detail, and Health Risk Evaluation Outcome to create Unified Health Risk
Details.
Health Engagement: Close Care Gaps flow that creates care gap closure outreach and
follow-up tasks for unified individuals based on risk scores and outreach engagement.
