MCG Integration with Integrated Care Management
The integration with Milliman Care Guidelines (MCG) enables care managers to access assessments based on clinical guidelines from MCG and create care plans for patients. MCG’s industry standard guidelines help users make informed decisions on patient care, resulting in improved outcomes such as reduced length of stay and higher independent quality ratings.

In Integrated Care Management, users can access MCG assessments from the Assessment component. When you enable MCG Care Guidelines, the Assessment component displays a slightly different interface from the standard component for Discovery Framework-based internal assessments. You can access internal assessments, external assessments, and suggested assessments from the component. You also get more options to quickly search and filter the assessments you’re looking for.
Some MCG assessments have clinical annotations, citations, and references linked next to the questions. Care managers can easily check the clinical source or reference for a question while administering the assessment. Having crucial information handy helps care managers save time and ensure credibility of the assessment.
The QM tag in some MCG assessment questions indicates that the question is important for quality metric reporting and shouldn’t be skipped.
Similar to internal, Discovery Framework-based assessments, users can create care plans from completed MCG assessments using the OmniScript care plan creation workflow. MCG recommends problems, goals, and interventions based on the responses to the assessment. These care plan elements have the MCG icon next to them to indicate that they’re MCG recommendations. Users can add more problems, goals, and interventions from the care plan interface, but they aren’t tagged with the MCG icon.
There are different intervention types that come from MCG, such as SEND intervention that users can download as a PDF and send to patients.
The Data Model
Integrated Care Management uses new and existing objects in Health Cloud to model data that comes from the MCG system.
After you administer an MCG assessment, the assessment’s metadata is stored in the External Assessment Definition object in Health Cloud. The external assessment itself is stored in the Assessment object, similar to internal assessments. The assessment question is represented using the Assessment Question object and the selected response is stored in the Assessment Question Response object.
When you create care plans from MCG assessments, use the FHIR R4-aligned Care Task and Care Task Details objects to represent subinterventions. Problems recommended by MCG are mapped to Health Condition, while barriers are mapped to Care Barrier.

To access MCG assessments and care guidelines in Integrated Care Management, enable MCG Care Guidelines, configure remote site settings, create a named credential, and create a permission set. Finally, connect Health Cloud with the MCG system by creating an integration definition. - Add MCG-Related Components to Record Pages
To enable care managers to administer MCG assessments and create care plans from completed assessments, place the Assessment component and care plan interface on record pages. To track the history of an MCG assessment, place the Assessment history component on the assessment record page. - Set Up Prefills for MCG Assessments
Set up prefills for MCG assessments by turning on Prefill MCG Assessment Questions in Setup. Then, configure the disclosure for prefilled assessments and specify the look-back period. To enable community users to complete MCG assessments on Experience Cloud sites,add the External User Assessments component to your site, configure the external assessment integration defintion for MCG assessments, and assign the required permissions.
