Update existing care plans by adding new problems, goals, and interventions using
Discovery Framework-based internal assessments, MCG assessments, and predefined care plan
templates. You can maintain a single, comprehensive care plan for a patient, instead of creating
a new care plan every time you want to update the components. This makes it easier to manage
patient care.

To use an assessment:
Read and View All Records permission on Discovery Framework objects
Industries Assessment permission set license
To update a care plan:
Create permission on Care Plan and Case
Read, Edit, and View All Records permissions on Care Plan Templates, Care Plan
Template Problem, Care Plan Template Goal, Problem Definition, Goal Definition,
Problem Goal Definition, Action Plan Template
Before you start, create at least one care plan and complete at least one assessment.
On the care plan interface, navigate to the care plan you want to update and click the
dropdown button.
Click Update Care Plan.
Select Assessments or Care Plan Templates as
the method for updating the care plan.
If you selected Assessments, specify the assessment type you want to search for. From
the Search Assessments In dropdown, select Internal
Assessments to search Discovery Framework-based internal assessments
configured in Salesforce. To search for external assessments based on MCG guidelines,
select MCG Assessments.
Select at least one completed assessment or up to four care plan templates.
Click Next.
Enter a name for the care plan.
Select the start date and end date.
Compare the recommended problems, goals, and interventions based on the assessments you
selected to the current problems, goals, and interventions in the care plan.
From the recommendations, select the problems, goals, and interventions you want to add
to the care plan.
The recommendations that you select are seamlessly merged into the existing care plan
hierarchy.
Save your changes.
