A common SDOH example is food insecurity. When patients and members don’t have an adequate
number of meals or snacks daily, or the quality of their meals is poor, this can lead to
higher-than-average visits to the emergency room. The intervention is to provide access to
affordable nutritious food via food debit cards, food banks, food delivery services, farmers
markets, and subsidized meal programs.
Grant Access to Records for Social Determinants Use sharing settings to specify the level of access your users have to records and fields used to assign and track information about social determinants. Organization-wide sharing settings specify the default level of access users have to each others’ records. You use organization-wide sharing settings to lock down your data to the most restrictive level.
Customize Picklists and Fields for Social Determinants You can specify picklist values that are meaningful to your organization and users for the Care Determinant Type, Care Barrier Type, and Care Intervention Type objects. If your company uses custom values for the Status and Priority fields, you can add or change them as well.
Add the Care Determinants Component to a Record Page The Care Determinants Lightning component lets you associate barriers with an object like an account and configures how barriers and their details appear in the app.
Create Care Barrier Types Create a library of standard types or classes of barriers that patients or members encounter. Access to transportation is considered a care determinant. Lack of transportation is a care barrier type. Make sure to modify user permissions so that users have access to these types.

Use Social Determinants to Assist Patients and Members Providing great care means having a comprehensive, holistic view of the people you’re managing—including insights into their social and environmental circumstances. If someone with health issues can’t drive to the grocery store or to medical appointments, it’s difficult for them to maintain or pursue optimal health. Health Cloud gives you tools to assess and record barriers to health and address them with interventions that make the difference between struggling and thriving.
