          Create a Survey to Collect Feedback

          Collect feedback and information from healthcare professionals.

          Required Editions

          Available in: Lightning Experience
          Available in: Enterprise and Unlimited Editions with Life Sciences Cloud, Life Sciences Cloud for Customer Engagement Add-on license, and the Life Sciences Customer Engagement managed package.
          User Permissions Needed
          To create surveys: Life Sciences Commercial Admin

          Here’s a video to help you get started.

          1. From the App Launcher, find and select Surveys.
          2. Click New, and enter a name.
          3. Select Offline Mobile Survey.
          4. To use the survey as a template, select Create survey as a template, and then click Continue.
          5. (Optional) Edit the survey’s Welcome page. To hide the welcome information, click the eye icon. The surveys welcome message appears only on the web.
          6. In the survey builder, if you want your survey to have multiple pages, click Add Page.
          7. To add a question to the page, click Add Question.
            1. Enter your question and, if necessary, a description.
            2. If you’ve selected the question type as Multiple Selection, Picklist, or Single Selection, enter a choice, and to add more choices, click Add Choices.
            3. If you’ve selected the question type as Rating, then select the type of rating.
              There are five rating types: Star, Static Emoji, Thumbs Up, Heart, and Dynamic Emoji (define the rating scale).
            4. If you’ve selected the question type as Short Text, then select the Response Validation Type: No Validation, Number, or Custom (enter the formula in the custom field).
            5. If you’ve selected the question type as Slider, enter a minimum value, interval, and a maximum value.
          8. To reorder the questions, use the up and down arrows.
          9. To change the question’s properties, click the Question property settings icon.
          10. (Optional) Control which page a participant sees next. See Show Survey Pages Based on Merge Fields.
          11. Add content to the survey’s Thank You page. The Thank You page appears only on the web.
          12. To review the survey as you work on it, click Preview.
          13. When you’re done, activate the survey.
