Use Medication Management

Health Cloud helps you reduce medication-related errors by providing you with the tools you need to develop a curated list of current medications; review medications with the patient; make clinical decisions and recommendations based on the review; and communicate the recommendations to the patient and the patient’s prescriber.

• Medication Reconciliation
  With more than 40 percent of medication errors believed to result from inadequate medication reconciliation, it’s critical that you perform a thorough medication reconciliation during transitions of patient care. Medication reconciliation is a comprehensive evaluation of a patient’s medication regimen achieved by comparing the patient’s current and previous medication lists. The process helps identify if the patient is taking any new medication, using non-prescription drugs, or has changed previous medications.
• Reconcile a Patient’s Medications
  Efficiently manage and reconcile a patient’s medications list using the Medication Reconciliation tab of the Patient Medication Manager lightning component. You can easily review the patient’s medication information, identify errors, and add recommendations and notes, all from one place.
• Manage Medication Recommendations for Patients
  Use the Recommendations section to see a consolidated list of recommendations that you added during the current medication reconciliation. From here, you can create, change, or remove a recommendation, or add more specifics to a recommendation such as notes for the provider and patient.

  Medication Therapy Management offers a broad range of healthcare services to help patients optimize their medication regimens for the best possible therapeutic outcomes and reduced healthcare costs.
• Conduct a Medication Therapy Review For Your Patients
  Work together with your patients to improve their knowledge about their active medications and empower them to take ownership of their health with medication therapy reviews. You can conduct either a comprehensive or targeted medication review for your patient.
