          Create a Care Plan Template

          Create care plan templates with benefits, goals, and tasks that you can easily reuse to connect individuals with standard types of services and support. Use care plan templates to deliver benefits consistently and expediently. Talk to your Salesforce admin about being able to clone care plan templates.

          User Permissions Needed
          To create care plan templates:

          Care Plans Access permission set

          OR

          Education Cloud Full Access permission set

          A care plan template is a blueprint of benefits, goals, and tasks to help individuals improve their circumstances and achieve positive outcomes. Create care plan templates to address health, safety, well-being, and other personal and social issues. For example, create an employment assistance care plan template that caseworkers and other staff members can use to accelerate the development of an individualized care plan for an unemployed individual. When you create a care plan from a template for a specific case, you can use it as is or customize it for a particular individual’s needs.

          1. From the App Launcher, find and select Care Plan Templates.
          2. Click New.
            Talk to your Salesforce admin about creating a button to clone a care plan template and its related care plan template goals, care plan template benefits, and action plan template assignments.
          3. Enter a name for the template that reflects the assistance scenario.
            For example, Stable Employment or Healthy Household.
          4. Select Draft as the status.
            Note
            Note You can’t edit a care plan template after it’s published. Don’t publish the template until you add goals and benefits.
          5. Save your work.
          6. On the Care Plan Template Goal related list, add goals.
            • To add one goal, click New. Select a goal definition, set the priority, and save your changes.
            • To add multiple goals, click Add Goals. Search for and select the goal definitions. Adjust the priority, and save your changes.
          7. On the Care Plan Template Benefit related list, add benefits.
            • To add one benefit, click New. Select a benefit, set the priority, and save your changes.
            • To add multiple benefits, click Add Benefits. Search for and select the benefits. Adjust the priority, and save your changes.
          8. To add tasks, go to the Action Plan Template Assignment related list, click New, and then select the action plan template version.
          9. To see the hierarchy of the template’s goal definitions and benefits, go to the Preview tab.
          10. When you’re satisfied with the care plan template, publish it.
            1. Click Show More icon, and select Edit.
            2. Select Published as the status.
            3. Save your changes.

            Note To create a care plan from a care plan template, the template must be published.
