          Case Referral Intake

          Fulfill your mission to serve your community by effectively processing referral requests to match individuals with support services and social and educational programs provided by your organization or by an external provider. Use guided flows to quickly submit referral requests and create cases for them. And use them to collect information from individuals and determine the programs and services that can help them. Then, create cases from submitted referrals that caseworkers or other staff members can use to refer individuals to providers that offer the programs and services that meet the individuals’ needs.

          Help deliver support services and social and educational programs to individuals by fielding requests for referrals. Collect important information from the person who needs help so you can quickly connect individuals with programs and services.

          For example, use Case Referral Intake to:

          • Help unemployed individuals get into workforce development programs.
          • Connect adults with positive parenting programs.
          • Help families enroll their children in early childhood education programs.
          • Connect older adults to meal delivery services.
          • Connect developmentally disabled adults with independent living services.
          • Refer university students to mentoring, financial aid, advising, or disability services programs.
          • Refer veterans who have a disability to tuition fee-waiver programs.

          Case Referral Intake uses these terms.

          Client
          An individual who seeks a referral to a service or program offered by a specialized provider. For example, an unemployed individual who seeks a referral to a job training program.
          Referral Request
          A form that an intake agent or officer completes to request a program or service managed by the intake agency or a specialized external provider.
          Referrer
          An organization or individual who refers someone to a service or program. For example, a nonprofit that provides job training programs receives a referral request from a state or regional employment development department for an unemployed individual (client).
