Create a Case Referral Intake by Using a Guided Flow
Use a guided flow to receive, process, and assess requests from individuals and organizations for referrals to services, programs, and providers.

Use the case referral intake guided flow to efficiently process requests to connect individuals with support services and social and educational programs. Quickly capture referrer and client details, complete assessment questions, and attach supporting documents so that you can give all of the necessary information to the service provider when you make the referral.
- From the App Launcher, find and select Referrals.
- Click New Guided Intake.

Specify the referral intake details.
- Enter a brief description of the referral as the title.
- Select the origin of the referral request as the source.
- Select a category and a priority.
- Select a status and a result category.
- Select Self or Someone Else as the referred person.
- In Referrer Details, search for and select an account or contact, or enter a referrer name and contact information.
- In Client Details, search for and select an account, or enter a client name and contact information.
- To ask additional assessment questions, select Add Other Documents and Assessments, and then select an assessment type.

Click Next.
If you selected Add Other Documents and Assessments and an assessment type, these steps appear: Upload Documents, Assessment, and Review Assessments.

On the Upload Documents page, attach a photo ID, proof of address, or any other supporting
document. To request supporting documents, enter an email address, a subject, and a message
body. Then, click Send a Request.
Note After you upload a document, you can’t go to the previous step and deselect Add Other Documents and Assessments. To deselect Add Other Documents and Assessments, first remove any uploaded documents. - Click Next.
- Complete the assessment questions, and then click Next.
- Review the assessments, and then click Next.
- Click Done.
The flow creates a referral record with the attached supporting documents, an Interaction Summary for the referral, and related assessment records.
