Provide Assessments to Analyze a Patient's Health
Health Cloud Assessments use the power of Discovery Framework and Omnistudio to build more complex questionnaire paths. You can complete assessments in two different formats. A healthcare professional administers assessments such as screeners or questionnaires over the phone or in person. Send an email notification to a patient with a link to access assessments as a guest user on an Experience Cloud site.

- Get Your Org Ready to Use Assessments
To enable Assessments, you must enable Discovery Framework and update sharing settings. - Deploy Sample Assessments
Start or expand your library of assessments with prebuilt samples for healthcare. The samples illustrate common, standard healthcare interactions. If you’re new to assessments, the samples orient you to the typical configuration and use of assessments in a healthcare setting. - Create an Assessment with Discovery Framework
Create an assessment form using the OmniScript designer in the OmniStudio app. The Assessment Component shows assessments that you create using the Discovery Framework. - Prefill Health Cloud Assessment Questions with Previously Submitted Responses
Save the effort of entering or requesting the same information multiple times. Improve patient interactions by verifying auto-populated data from previous assessment responses and focusing more energy on the patient’s current concerns. - Give Users Access to Assessments
Create a permission set that enables users to view and complete an assessment. - Add the Assessment Component to Record and App Home Pages
To give care coordinators like intake and behavioral health specialists quick access to assessments, add the Assessment component to pages. Record pages show assessments related to a particular patient, and app home pages show assessments for all of a user’s assigned patients. - Set Up Assessments for an Experience Cloud Site
Add the assessment component to an Experience Cloud site. - Update an Existing Assessment for Patients to Complete on an Experience Cloud Site
A Discovery Framework assessment created before Summer ‘23 is missing a remote action that is needed for it to work on an Experience Cloud site. Edit the existing Omniscripts you want to use on an Experience Cloud site to include a remote action and key value pair to set the usage type. - Give External Users Access to Assessments
To show external users on an Experience Cloud site the assessments assigned to them, create the necessary profile, permissions, and object access. When patients complete assessments at their convenience, healthcare practitioners can focus on providing care and minimize the time spent on gathering information. - Set Up Score Evaluation
Configure score evaluation for assessments so that users have quantifiable data to support their understanding of a person’s responses. Add the Response Value Score field to the Assessment Question Response page. Then, set up a score evaluation tool and configure the Get Assessment Response Score flow to meet your unique business requirements. - Set Up Assessment Score Tracking and Analysis
Evaluating and monitoring assessment scores with the Assessment History and Assessment Questions components helps users analyze important changes over time. With data to support their understanding of a person’s performance and progress, users can confidently make decisions, plan timely interventions, and eventually improve outcomes. - Set Up Suggested Assessment Responses
To help care managers access smart suggestions to assessment questions, configure suggested responses for Discovery Framework-based, internal assessments. - Configure Autosave for Assessments
Enable autosave for assessments so that users can administer assessments without worrying about data loss. As users enter assessment responses, the assessment is automatically saved every few seconds. - Set Up Unified Search Experience for Assessments
To enable users to find both Discovery Framework-based internal assessments and external assessments from a single search bar, set up Unified Search Experience. Users can find Unfied Search Experience in the Assessment Library tab of the Assessment component. - Enable Historical Access for Internal Assessments
To enable users to open completed Discovery Framework-based internal assessments and view historical responses within their original context, enable Version-Aware Historical View for Internal Assessments. This view helps users gain a better understanding of a patient’s health journey by accessing completed assessments, regardless of their current version. - Set Up Generative AI Suggested Assessments
Set up Generative AI Suggested Assessments to provide your users with AI-powered assessment suggestions based on a patient’s profile across Agentforce Health, Data 360, and zero-copy external sources.
